If you genuinely believe that you tried your best then no, it's not your fault. The only times I'd say it's someone's fault is if they were an abusive jerk who knowingly dismisses their kids' or loved ones' feelings while adding to their pain etc...But there are times when you just can't do much, or there isn't much you can do to begin with.
