So ego-syntonic is when a thought or behavior aligns with someone's core values while ego-dystonic is the reverse of it. Someone with ego-syntonic SI might be treatment resistant because, well, they dont think anything is wrong with that ideation. Might not even feel distressed, at least for me.

How do they help with getting rid of something said person doesnt feel the need to get rid of?

That's a very vague question. The mind is an extremely complex system; you may not even be aware of the reasons behind a particular thought, the mechanisms behind its formation, or the function it serves. A lot will also depend on the specialist you're working with.
