That second page makes me nervous "What I DO NOT want at the end of my life (check all that apply)" because I can't help but interpreting it as that the person does not want to be pain free, does not want to get help to die gently and naturally and does not want to die at home.

I have one of those all filled out for my kiddo. The front cover says "I'm DEAD. Now all this fucking shit is YOUR problem!" Yeah. We both have a fairly dark sense of humor.

This reminded me that I still need to prepare my advance instructions document and present it to the government so they know what to do with me, in case something goes wrong with my attempt.

I made a will and transferred all my property home and vehicles already. months ago. any day will be my day, or I wait till June when I can get far into the wilderness so I am never found. every moment is hell so it's a daily toss up. tragic.

It's definitely cute, but I doubt that something like this will prevent the demons from trying their hardest to keep you alive.
