In your opinion, what is the worst incurable illness (and please explain your reasoning)?
For me personally, it has to be the 2 variants of the Herpes Simplex Virus. Both are quite common, HSV2 (the lesser common of the 2) still affects 1 in 6 people. Most people are asymptomatic and never experience symptoms, so they never feel the need to get tested, and if sexually active, pass it on to their partners unknowingly. Antivirals only help to more-rapidly heal outbreaks or reduce the frequency of occurrence. Any skin to skin contact can infect your partner, even absent an outbreak. Viral shedding means you can still infect someone or spread it when you think you are fine. Condoms do not fully prevent transmission. Ocular herpes can ultimately lead to blindness.
A cure needs to be developed, because this shit is hell, but science "supposedly" doesn't understand it enough... ugh!
