It's because they don't angle it downward. If you search up "ligature suicide" on google you'll notice the marks are angled downward. This is the reason why people say they put the noose below he adam's apple. It's because it already assumes a downward angle. That way you're hitting the carotids. So even if you put the ligature above the adam's the apple if you angle it you should be good to go.
at leas that's what I think.

So if there's anyone worried about using the method like I am it's autoerotic asphyxiation. There have been successes.

Just don't test it out until you're absolutely ready to die. Messing up can give you brain damage and problems. Same goes for partial/full hanging.
