Good point. If we were immortal til we decide to stop Or if life was composed of constant orgasms and marshmallows and we suddenly decide to die then it would be the whole other story.
Alas, life is crap and we are mortal suffering parasites with sick imagination relishing those rare moments of relief from reality.
Fear of the unknown and pain (our and close ones) is holding us back, I reckon.

I read one user saying it's the last decision we'd make. Thinking about it that way, we need to be 100% certain that's what we want.

I know very well that it's hard, but I believe aggression is the key. If you're pissed of enough about the world just like I am, nothing will be able to stop you, including hanging (pun not intended) tight to your plan to CTB. Good luck marine!
This is indeed a good question. I'm sure the answer is rooted in biological survivalist tendencies. No other species (as far as I know) *tries* to die. Others die willingly in the process of completing a task, but I can't think of another that intentionally kills itself just to stop living. Does anyone know of any research on this?
This is my new favorite phrase.

Not that I am, but I heavily believe it. Physics and the materialistic sciences plus evolution point towards not having a soul, an that our conciousness is merely a product of our atomic constellation, so when we die, these processes break down and we are no longer. So I personally believe it's EXTREMELY unlikely there's anything beyond life, and if there is, ffs it can't be worse than this hell of an existence.

It is a very big decision because you are going against what your biological instincts and everything that you were brought up in (indoctrination by society, morals and values, ethics, etc.). As for the biological instinct alone, this self-preservation instinct (called the 'SI' - survival instinct) is such a powerful force to overcome such that most people who may want to CTB, may get hindered, if not outright stopped by that alone. Then of course, assuming one overcomes the SI, one has to ensure that his/her method will work (done the research, acquired the materials, practiced enough to ensure reliability and execution), and finally, hide it from everyone in the world that is looking to intervene. All of these factors make CTB'ing a big decision.

Honestly, it makes more sense to me that 'the last decision you'll ever make' would mean that it's an even less significant decision. If death is so final that you can't even make decisions after it, then there's no room for regret. It would not matter one bit to me if I was sniped and instantly killed right now, even if I was the happiest person in the world, because of how final death is. I know it's an extreme view for most, so in casual discussions I still advocate to think death through and be sure and all that, but when it comes to me personally, death cannot be 'rushed', as rushed would imply death can come too soon.
