Over the past year, increased regulatory pressure in multiple regions like UK OFCOM and Australia's eSafety has led to higher operational costs, including infrastructure, security, and the need to work with more specialized service providers to keep the site online and stable.

For the binding agreement you have to talk to the doctor and need it signed by a lawyer. It's also only valid for 8 years and also becomes irrelevant should medical treatment improve after you signed it.
If you're in Germany, then the doctors have to follow your PatientenverfĂĽgung. It's valid forever as soon as you sign it. (Unless you revoke it or you write valid until date xx.xx.xxxx in it)
