Jesus, they all screwed up somehow. I don't know why it is so hard for people to just follow the precedent set by the guides here or in the PPeH.

Problem is that believe it or not, survival instinct can play a role in this, people's thoughts can be all over the place and therefore may be detrimental to using the protocol as effectively as possible.
