SI: you back out before you pass out, practice and getting in the right mindset can help. Rope or anchor fail: these are very common, test your rope and your anchor to see if it can hold your weight for extended periods of time beforehand. Position: your position might take some pressure of the neck causing blood flow to be restored and you wake up. make sure you get the correct position for your location. (look at other threads for this) Someone else can come in while you are passed out and save you: try to have at least 30 mins alone to prevent this. These are all the scenarios i could think of (i might have forgot some things)
