From my research here it seems like most failed attempts are due to being rescued/call for help. Not saying that it can't fail anyway, but I haven't seen any threads like that.
Three major factors to SN failure (from what I have seen on this site):
Getting help, whether asked for or not.
Low SN purity. Either an unreliable vendor, accidentally buying to the one pre-made for meat curing (only 6% not 95+%), or accidentally buying sodium niTRATE instead of niTRITE.
Not following the protocol. Not accurately weighing the SN. Not fasting.
