My wish and decision to die are based in multiple grounds.

physical, psychological, emotional abuse and complete emotional cutoff from both parents and sibling -since childhood throughout almost entire my adulthood
traumatic experiences excluding familial abuse and abandonment
complete isolation - no family, no friends, no social activity at all
lifelong existential, situational depression, possibly ptsd after going through multiple traumatic events
a couple seemingly chronic physical symptoms

I'm worried I'm not able to obtain sufficient records to get approved for VAD since most my mental health treatments and traumatic events occurred almost two decades ago. I feel it is just nearly impossible to gather documents that materialistically prove my life long suffering. If I get rejected by Pegasos, could I reapply later? Should/could I write a personal statement about my life history and life-long suffering to convince them? Would it help? Or I wonder if I should set up an appointment now to get a doctor's note.

Yes, you need to start collecting documents from professionals to convince them, not personal statement.

Yes that's what my friend was doing; getting records and scanning them to send. Her psychiatrist had provided very good records.

I don't see anything about retrying if you are rejected but if you have new records or your condition worsens, I would certainly think you could reapply.

Indeed, I have the problem, in Switzerland, with another association, of having to be accompanied by a relative for example, despite having obtained the green light. Without accompaniment by someone, I cannot benefit from the VAD, despite the validation of the file by green light.

this article says a US woman named Krista Atkins, paid "additional $2,500 to Flemming Schollaart, founder of the Right to Die Society in Denmark, who they say drove from Denmark to Zurich airport to meet her and served as the witness—a requirement by Pegasos".

Krista Atkins was known to have a severe depression and was 40 at the time of death. Furthermore, it says "Atkins' scheduled death happened at a time when international travel was restricted by COVID-19, which raises questions about how she was able to get permission to travel to Switzerland".

Two AZ sisters brought a guy with them to Switzerland and he allegedly received the death certificates from US embassy there.
