How do I get afflicted with a terminal disease to CTB?

I planned to do a CTB combo earlier in another thread, but then decided against it, since SI is too strong for me to follow through with some of the steps, so now I'm here to ask for a new method regarding the title of this thread.

I decided that I wanna live, but at the same time have a guaranteed chance to auto-CTB at any moment in the next 1-5 years since I'll just never escape the demons that has been haunting me ever since I was born.

Let me auto-CTB unexpectedly while I lived as happily as I could beforehand with no worry about it.
