I'm sorry that a chain of shitty events made your life even worse. I would suggest having your leg heal first and foremost, then the next immediate thing is to get out of the psych ward as quickly as possible (before it makes your mental state even worse) as that would allow you more privacy and freedom to plan your next steps/actions. As for the SN, I think it's pretty much gone sadly 
until you are able to get a new one (I have no knowledge or advice on how to go about that as that isn't my method nor where I know much about it).

Be easy on yourself, friend. If we did'nt do dumb things we would'nt be human. Why institutionalize yourself, dear? Just recover...relax at home...focus on things you can do if possible. Watch that leg, yes?
By the way, friend, how is that leg doing? You May think your dumb, but believe me, dear, i've Done DUMBER
To avoid psych personally I would just say that the injury to the leg gave me a new perspective on life and I no longer wish to die. That I did indeed have plans but now that I have faced my mortality I am happy to be alive.
Great news! Facing mortality makes you stronger, not weaker. I know that i don't know you but I know that I love you. I know it must sound like just another platitude, but genuinely felt. Glad that leg is doing better! Keep on keeping on!
God damn it, my anxiety got better while I was in hospital and led me to believe all was good, and life was great, but now I'm home its flared up again and the suicidal thoughts are back. I just want to make it stop!!
