These guys are nutjobs, because they're REALLY hitting each other with those weapons as hard as they can. The only part that's faked is the part where he executes the loser; it's alright to injure your opponent, but intentionally killing them is a no-go.
The rules are that you keep fighting until one person surrenders, one person is in too much pain to keep going or you manage to land a hit that would be lethal. People have died doing this before, and many more have gotten hospitalized.
It's an extremely expensive sport to get into; you have to purchase your own armor and weapons, and they are not cheap at all... I'd do it if I had the funds, though.

That's lances for you. They were a deadly and commonly used weapon in late-stage European warfare for a reason. As the horse charges forward and you thrust the lance into the opponent, the sheer kinetic force generated from the charge is transferred into the tip of the lance, which then disperses into the target's body.
