CO method

CO is my chosen method for CTB.
Have a pretty bullet proof plan, but am looking for flaws.
Anyone have failed attempts at CO, if so would you be willing to share what you think went wrong ?

As a commercial diver I absolutely understand, no O2, no life.
Mainly looking for things that went wrong for others just in case I've missed something.

CO poisoning is rather a general principle than a method. There are entirely different implementations of CTB via CO, depending on how CO is obtained and how it's inhaled. For example, you can produce CO via a chemical reaction between sodium formate and concentrated sulfuric acid, collect the gas in a plastic bag and then inhale it from there. Or you can make CO via incomplete combustion of charcoal inside a small room and try to poison yourself by entering the room. Obviously, these two methods are very different from each other despite the common CO poisoning part.
