I remember a friend of mine in a country in Europe I can't mention get much better access to them through a industry that sells them and another from South Asia through friends (opiods), but some parts of the world, it's pretty much hard, and even though drug dealers lace their products, it's pretty much always hard to get the real deal. Why's that? I can't even find any dealers who would even want to sell them let alone give you opiods, unless it was another why and that's usually always through prescription alone.

They are incredibly addictive and deadly. Generally, you want to keep the population safe so you make illegal things that can kill people or get them very addicted which means they hurt others to feed their addiction (ie, shoot someone in a robbery to get drug money). Because these are so deadly, the punishment for selling them is often very harsh, so many dealers will not touch them for fear of longer/harsher sentences if they get caught.

In other parts of the world, policing of addictive substances is not as good. It could be lack of resources, corrupt officials, or a government that just does not care if a bunch of its citizens are killing themselves. Also, many opioids are manufactured in Asia, being closer to the source means less need to smuggle them across multiple border, means less likely to get caught and also potentially cheaper prices.
