It's when part of you still makes contact with the ground, slumping forward, such that body weight creates pressure so the rope compresses the—I want to say carotid. Lack of blood flow to brain causes death. That's my understanding.

It's usually done in combination with pills/alcohol, etc, to overcome what ur talking about. It would be painless, yes—it's like a sleeper hold, you just pass out. I imagine you'd feel pressure in your head, but nothing insurmountable. Now I haven't tried it beyond seeing what it might feel like—I think it's doable, and quick if done right.

I rather doubt such a thing would be painless. Suffocation seems panicky.
