I do believe you light it up in an open area and wait for the fire to die down and the coals are doing the work at that point.

Also, you want to breathe carbon dioxide, not the smoke, which will irritate eyes, mucus membranes, and lungs. From what I understand, more CO is released after burning charcoals than during, so if you have no choice but to burn indoors, then do it in a different room than the one in which you plan to ctb, However, if it's in an apartment, there's a chance neighbors will smell smoke. It's just not a good idea to burn indoors.

When you start a fire in a closed area, you won't be able to be there more than a few minutes. Smoke is irritating everything that it can irritate so the fire should be started outside prior to CTB (I mean burning the coals until they are gray). When you are in the room full of smoke, you won't make it. Bad cough, sore eyes and cursing the method which did not work.
