Sodium nitrite: not your cure-all remedy
A 52-year-old Caucasian male was transported to hospital via ambulance after being found at home by family, unconscious and slumped awkwardly for approximately 24 hours. On day 2 the patient divulged purchasing sodium nitrite online and self-administering after reading a website dedicated to euthanasia. Psychiatry reviewed and withheld his regular psychiatric medications. Methaemaglobin was found to be 2.5% (reference range 0%-1%). Toxicology was consulted and felt that sodium nitrite was not the cause of his organ dysfunction. Haemodialysis was started on day 4 to treat his acute kidney injury (AKI) from haempigment nephropathy cause by his long lie prior to admission. On day 17 the patient was unable to retain warfarin counselling information on sequential days. A Montreal Cognitive Assessment (MoCA) was performed, scoring 13/30, with poor executive, visuospatial and verbal fluency. It was unclear if this was premorbid, but it was felt past history of ECT could have contributed.

A University of Southampton student died after consuming a fatal amount of sodium nitrate, an inquest heard. Wira Wahyudi Sinuraya was found unresponsive on his bed by his housemates on February 14th this year. The 21-year-old was pronounced dead at the scene by emergency services, who were called to his home on Honeysuckle Road. At Winchester Coroner's Court on Monday this week, an inquest into his death heard how a toxicologist found the level of nitrite in the student's body was over 100 times the expected concentration.

Autopsy: Medical examiner rules Florida Polytechnic University student killed himself
LAKELAND — The death of a student found last December on the campus of Florida Polytechnic University has been ruled a suicide. Dr. Stephen Nelson, medical examiner for the 10th Judicial Circuit, determined that Joshua Barton intentionally ingested sodium nitrite, an inorganic salt used as a food preservative. Barton, 29, was found in a wooded area on the Lakeland campus on Dec. 6. Nelson performed an autopsy the next day, and his office sent samples to the University of Florida Pathology Laboratory, which forwarded them to a laboratory in Pennsylvania for a toxicology analysis. The Medical Examiner's Office received a report from UF this month.

Also according to information given to the Civil Police, the professionals of the health unit who were there did everything to save the life of the same, without success, afterwards they died. Sodium nitrite, a preservative authorized only for the conservation of meat, was present in the sorbitol E420 taken by the girl, as communicated last night by medical sources then confirmed by judicial sources: the substance had been purchased - probably in Ireland - in a pack of 5 chili like sorbitol on eBay. However, a package that contained sodium nitrite with a concentration of 70 percent. The toxicologist of the Poison Control Center of Pavia Carlo Locatelli explained that sodium nitrite is a very common substance, used as a preservative for food, with the same external appearance as sorbitol, with which it can therefore be easily exchanged. "Sodium nitrite, also known as saltpeter, is quite common, and in microdoses it is used as a preservative for food - explains Locatelli - given the speed with which the substance had acted in the case of the Barletta woman, one of the most probable hypotheses was just that it was this molecule." Nitrite has the appearance of a white powder, and the lethal dose for humans is set at around 22 milligrams per kilo of body weight. Its ingestion interferes with the transport of oxygen by the hemoglobin. It has antibacterial properties, which are exploited in the preservation of food, although recently some studies have shown a long-term carcinogenic effect of this substance.

A man from the Midlands has been found dead alongside a woman in an AirBnb cottage. The double-death tragedy saw the 28-year-old male's body discovered with the female, aged 24. Police in Scotland are investigating the double-death of the Stoke-on-Trent man. Brett Stevens and the 24-year-old woman were found at the holiday cottage, reports StokeonTrentLive, our sister title. Police Scotland are not treating their deaths as suspicious and investigations continue. Now an inquest has revealed that Brett was living in Stoke-on-Trent before his sudden death. The identity of the woman has not been revealed. North Staffordshire assistant coroner Sarah Murphy said: "Brett Stevens died on December 5 at a holiday cottage in Stonehaven, Aberdeenshire. "He was identified by police officers and the brief circumstances are that he was staying in a holiday cottage in Scotland. The presumed cause of death is sodium nitrate toxicity."

The past few weeks, her mother said she didn't notice anything wrong. There was no sign of what was to come. On that Wednesday morning, her mother, who works from home, saw her at about 11 or 12. She seemed fine. A couple of hours had passed and she hadn't heard from her daughter so she knocked on her bedroom door. She thought maybe Shawn was taking a nap. There was no response. She opened the door and walked in. Shawn was lying on the floor. She was blue. She was cold. She wasn't breathing. She started CPR and called 911. It was too late. Shawn was gone.

La Plata County Coroner Jann Smith said a welfare check conducted Wednesday found Ainsley dead at her home in the Forest Lakes subdivision, northeast of Durango. Ainsley, who had been dealing with health issues, had ingested a sodium nitrate powder mixed with water. She had died the same day she was found, Smith said. The death was ruled a suicide, Smith said. No autopsy is required, she said.

Ashley Walker died after taking a lethal dose of sodium nitrite at his home on Oaks Road in Kenilworth last August. The 25-year-old called emergency services and when the ambulance crew arrived he was unconscious. They provided Mr Walker with oxygen and said his levels of consciousness showed slight improvement as his limbs began to move. But the crew was told to leave the house by the ambulance operations manager who believed they were dealing with a spillage rather than the swallowing of the substance.

The last one I remember reading about that - it's a pretty bad fuck up there. I mean, he's kind of lucky those people were so bad at their jobs.

I think in the UK your chance of dying by SN is greater purely because of the incompetence of most emergency services personnel (thanks to cuts) and the fact they don't keep methylene blue stocked in ambulances here. So, they show up after a really long time and then piss about getting you to hospital by which point you're basically dead anyway - fantastic. /s

Yeah, to not die especially in that first 24 hours, and to have such low methaemoglobin levels (barely above normal range) without being treated by methylene blue, would suggest he must have either taken far too small a dose and/or had other factors interfere with its absorption, such as not following the protocol properly. His very low cognitive function might have contributed to an inability to follow the protocol, although lord knows there have been enough members here who willfully ignore such instructions despite being intellectually normal.
