According to their website I will need:

- Reason for requesting a VAD
- Your civil status & current living situation
- A brief biography
- Family & support crew background
- Health reports
- Contact person nomination
- Birth certificate
- Proof of residence
- Marriage / divorce certificates
- Instructions for cremation, ashes etc.

What counts as proof of residence? I only have a passport, driving license and birth certificate. I don't get any bills in the post with my address. Maybe I could request paper statements from my bank if that's enough?

Also I wonder how long I'd have to stay in Switzerland for - since their fee is high I don't know if I can afford a long stay at a hotel there?

I also wonder how to travel there. Once off the flight, how do I get there? This is explained in their travel section

And how do I get a health report in the UK? Can I just ask my doctor for that?

Last but not least, it says I need a birth certificate issued within the last 6 months - how would I get that in the UK?

Birth certificate is easy .....just order it on line ..costs £ 15...OR £ 38 quick version...
