It is while partially suspended and they often put a lemon in their mouth. The idea is that you bite down on the lemon if you begin to blackout and it provides a mild resuscitation type response.
Never tried this as it's not my cup of tea. But I've read enough stories of this going wrong.
Drowning is probably the best method for the criteria here. That or maybe a car accident, but there's too many variables with that.

Yes, there was a UK MP years ago (name escapes me) found hanging after what appears to have been an autoerotic asphyxiation adventure, so not CTB, and he had an orange in his mouth. Not a lemon then but it seems to confirm the citrus fruit thing.
