Phenazapram is an interesting, very powerful benzo that is NOT a controlled substance in the UK or USA.

Phenazepam is a long-acting benzodiazepine that was developed in the former Soviet Union in the 1970s. It is not a scheduled drug here in the United States or in most of Western Europe. Phenazepam has become a drug of concern due to the number of hospitalizations and fatalities following overdoses. The drug is potent at one-tenth of the recommended dose for diazepam. The lack of data on this drug highlight the need for healthcare professionals to be aware of its existence.

In Russia, the indications for phenazepam are epilepsy, anxiety, and sleep disorders. Its increase in use for nonmedicinal purposes has been reported in Western Europe in the last couple of years, and its use has spread to the United States, with recorded deaths resulting from its use in Georgia and Louisiana. Some of phenazepam's concerning side effects include loss of coordination, drowsiness, and amnesia. I believe the latter side effect to be particularly concerning due to its potential for use as a date rape drug.

Fatalities have been reported when the drug is taken with other prescription opioid analgesic drugs. Phenazepam has been referred to as benzo-relaxation by its users.

As mentioned previously, phenazepam is not a controlled substance in the United States and in parts of Europe and can be obtained over the internet, where it has been sold either as a tablet or as crystalline powder. In the United States, it has been sold as an air freshener known as "Zannie." Reports indicate that spraying Zannie into the mouth is the main route of administration, and when used with antidepressants, sleep medications, pain medications, or alcohol, it can prove fatal. The product Zannie was tested at the Louisiana Poison Center, which revealed that the product contains 100-percent phenazepam. As a result, legislators in Louisiana have filed a bill to make the product illegal in the state.

The detection of phenazepam in the blood stream has been demonstrated using gas chromatography (GC), gas chromatography/mass spectrometry(GC/MS), and liquid chromatography/mass spectrometry (LC/MS), but there are no data regarding the sensitivity of available immunoassays.

The need for healthcare personnel to be aware of phenazepam, its symptoms of use, and its risk of death by overdose alone or by use with other substances cannot be overstated, particularly in emergency room settings. 🤗🌹💔

Xylazine is a tranquilizer, pain reliever, and central nervous system depressant that is U.S. Food and Drug Administration (FDA)-approved for use in veterinary medicine. It has not been approved for human use. Illicit xylazine is often mixed in with illicit opioids, most frequently fentanyl, but it is not an opioid. Xylazine is also known as "tranq" or, when combined with fentanyl or other opioids, "tranq dope." Xylazine can be used by injecting, snorting, swallowing, or inhaling.

Xylazine can make people drowsy and cause breathing, heart rate, and blood pressure to slow to dangerously low levels. When people take xylazine with fentanyl, it is not clear if xylazine increases the risk of fatal overdose. However, extreme sedation from xylazine can cause people to stop breathing. Most overdose deaths linked to both xylazine and fentanyl also involved other substances, including cocaine, heroin, benzodiazepines, alcohol, gabapentin, and prescription opioid medications.1

While opioid overdose reversal medications (such as naloxone and nalmefene) do not reverse the effects of xylazine, public health experts recommend people give overdose reversal medications for overdose with suspected xylazine exposure, because most people are exposed to xylazine by taking fentanyl.

Over the past two decades, synthetic opioids have increased the threat of the ongoing opioid epidemic. Nearly 70% of all drug poisonings and overdose deaths in 2023 involved synthetic opioids, primarily fentanyl. Also, among these substances is carfentanil, which stands out – at this time – as one of the deadliest fentanyl analogues. This white, powdery drug closely resembles other substances like fentanyl or cocaine, but its danger far exceeds that of nearly any other opioid on the street.
Carfentanil is a chilling reminder of how the opioid epidemic continues to evolve and introduces new threats at an alarming pace.
What is Carfentanil?
Carfentanil is a synthetic opioid originally developed for veterinary use, more specifically to tranquilize large animals such as elephants. It is extraordinarily potent and not approved for human use. In fact, it is estimated to be 10,000 times more potent than morphine and 100 times stronger than fentanyl.
Because of its strength, a nearly microscopic amount of carfentanil can induce a powerful, and often deadly, effect in humans.
In an effort to increase drug potency and profits, carfentanil has been found mixed with other drugs or pressed into pills that look like prescription painkillers. This increases the likelihood of fatal overdose and poisoning deaths, especially among users who are unaware they are ingesting such a powerful substance.
Increasing Prevalence
DEA, local law enforcement, and first responders have recently seen an increased presence of carfentanil in the illicit drug market, which has been linked to a number of overdose deaths in various parts of the country. According to the CDC, deaths involving carfentanil increased approximately sevenfold – from 29 deaths from January to June 2023, to 238 deaths from January to June 2024. Carfentanil has now been detected in 37 states.

What You Need to Know About 'Tranq' (Xylazine)

What Is 'Tranq' (Xylazine)?

Xylazine (pronounced zye-luh-zeen), commonly known as "tranq," is a non-opioid medicine that veterinarians use to tranquilize (sedate), relieve pain, and relax muscles in animals like dogs, cats, and horses. It's not FDA-approved for use in humans. It's typically sold as a clear liquid under the brand names Anased, Chanazine, Rompun, and Sedazine.

Around the early 2000s, people started to inject it as a recreational drug. It's a depressant that makes people feel drowsy, slows their heart and breathing rates, relaxes their muscles, and slows their brain activity. It's usually combined with other drugs like fentanyl, cocaine, heroin, methamphetamine, oxycodone, or alprazolam. Combining tranq with other opioids boosts or extends the effects of these drugs, makes them more addictive, and cuts costs for those who make the drugs.


When it's mixed with heroin or fentanyl, it may be called "tranq dope." When it's mixed mostly with heroin and cocaine, it may be called "speedball." Other street names in the U.S. include "sleep cut," "anestecia de caballo," or "horse tranq."


Tranq can have dangerous, potentially life-threatening effects on your health. For instance, one side effect is open sores (necrotic skin ulcers) on your skin, usually on your arms and legs. These sores can get bad very fast – and they don't appear just where you've injected the drug. Because tranq can cause extreme and prolonged sedation and skin sores, it's also sometimes called a "zombie drug."


Here's everything you need to know about tranq and its effects.

Is Tranq Legal?
Tranq is not approved by the FDA for use in humans. But it's not currently a federally controlled substance under the Controlled Substances Act. That means it's not illegal to use by itself under federal law. You can get it with a veterinarian's prescription. But in 2023, the U.S. government took steps to restrict importing xylazine and the ingredients necessary to make it to crack down on non-veterinary uses of it.


Some states have put xylazine on their own controlled substances list. For example, as of 2023, the governors of Ohio, Pennsylvania, West Virginia, and Florida have all taken steps to classify xylazine as a scheduled drug in their states. This means that tranq may be illegal to sell or use without a prescription in your state.

How Is Tranq Used?
Tranq you buy on the streets is usually mixed into other drugs as an adulterant to lower the costs of making the drugs. It may also be added to other drugs to enhance or extend the effects or decrease the side effects of those other drugs. For instance, tranq can extend the effects of fentanyl, which tend to last a short time, compared to other opioids. The recreational drugs it's often mixed with include:
  • Fentanyl (most often)
  • Cocaine
  • Heroin

  • Benzodiazepines, such as alprazolam (Xanax) and lorazepam (Ativan)

  • Methamphetamines

  • Opioids, such as oxycodone (Percocet)
Tranq is typically used in the following ways:
  • Injecting either into your muscle or vein
  • Snorting into your nose
  • Swallowing as a tablet
Some people may also vape or smoke tranq, but there's not much information about that so far.


To help prevent an overdose, public health researchers think it's probably safer to snort it than to inject it.

Tranq Effects
How long do the effects of tranq last?



This depends on the dose, how you took it, and what it was mixed with. But the effects can last eight hours or longer.


Tranq side effects


Xylazine is a central nervous system depressant, so it can cause:
  • Slow breathing rate
  • Slow heart rate
  • Very low blood pressure

  • Memory loss

  • Cardiac arrest, which is when your heart stops beating, usually because of an electrical disturbance
Long-term effects of using tranq


Long-term use of tranq can cause chronic complications with many of your organ systems, including your skin. It causes wounds and ulcers on your skin. This is probably because it reduces blood flow around your body, especially between your organs and to your skin.


Eschar and ulcers


Eschar are pieces of dead tissue that slough off your skin. This can happen when you have a severe burn, gangrene, or a severe wound, such as an ulcer. Research so far shows that about 40% of people who use tranq get skin ulcers. These ulcers seem to start on your arms and legs, and you can develop them no matter how you use it, whether that's snorting, swallowing, or injecting. These wounds can get bad very fast, so you may wind up with eschar.

Medetomidine: what you need to know about the animal sedative turning up in opioid deaths

The opioid crisis, increasingly driven by synthetic opioids, continues to claim tens of thousands of lives annually in the US alone. Similar crises have arisen all over the western world.
The crisis has become more complex as powerful synthetic opioids like fentanyl, nitazenes and oxycodone are now being "cut" (mixed) with other drugs that slow brain activity, including animal tranquillisers.
The emergence of medetomidine as a new contaminant in the US illicit drug market signals a worrying development in this escalating crisis. When public health officials in Philadelphia first began testing for the drug in May last year, medetomidine was found in 29% of fentanyl samples analysed. Six months later, the drug was found in 87% of fentanyl samples.
Medetomidine, a drug approved only for veterinary use as a sedative and painkiller, has increasingly been implicated in illicit fentanyl-related overdose deaths.

Like xylazine – another veterinary tranquilliser recently detected in street drugs – it is believed that medetomidine is added to fentanyl to boost its euphoric effects and hence make it more appealing to users. However, medetomidine is even more potent and longer-lasting than xylazine.
An overdose of medetomidine can cause extreme sleepiness, very low blood pressure, slow heart rate, trouble breathing (respiratory failure), and even coma or death. This is because medetomidine rapidly depresses the central nervous system and slows the heart rate significantly.
When medetomidine is used with opioids, the risk is even greater because both drugs can slow breathing, and together they can make it much worse.

Overdose-reversing drugs
Police and paramedics use naloxone to reverse fentanyl overdoses. But if the drug is mixed with medetomidine, naloxone won't work because medetomidine affects the body in a different way.
