Hi SS-community

I'll try to make this as short as possible. I'm in a clinic ,(open ward, but I still can't leave) and I have to ctb quickly because I have psymed-induced brain damage (including PSSD) and I'm deteriorating quickly. I do have a rope but there's no sufficient fixture. So I think I have to go by the tourniquet method. Any suggestions how to pull this off safely?

Yeah I thought about preparing it all and then sort of hide under the blanket when they check in on me during the night.
