I'm afraid of not actually ODing and waking up with brain damage or something, then I might not be able to ctb again. Is this a real risk?

Yes, unfortunately. I have seen this first hand. Damage to the brain, eyes, kidneys, liver, digestive system, etc. All possible. It's why failed overdoses are so terrible in addition to the possibility of a forced stay in a psych ward and other consequences. Oftentimes intervention happens quite quickly (the person backs out) and serious long-term damage is prevented though.
