Have you tried professional help and meds before? You can always try them out first and reassess from there.

Yes had therapy and meds for nearly 30 years now

Yeah, I know how you feel. For myself I often see death as preferable to endless suffering. I wish you the best though as nobody deserves the suffering that life brings them.
