Im familiar with exposure and response prevention therapy but it's torture. How do i stop all compulsions when not every compulsion is irrational?

Some have suggested a physiological component such as Inositol (the old vitamin B8). You might want to research this.

A behavior approach can be to identify one repetitive action and intentionally select a frequency (such as every third time) to do it slightly differently. This helps establish an action as a result of your control. This perspective of control is a key component.

Often an almost subconscious decision is made that some action is "necessary" and if not completed, anxiety arises with associated discomfort. By retaining and exercising conscious control, actions can be less a source of anxiety alleviation and more a source of anxiety prevention.

cloroprimime however it is spelled is recommended medication (after trying ssris). psilocybin might be worth a try.
