Topic preface: Whenever I start a topic here I'm going to start with this, and I hope it's okay. Why? because I see both the good and the bad things with this forum.
The good thing is that you can avoid mistakes which could result in worse conditions than you where in before your ctb, the bad thing is that someone can make a bad decision for a temporary problem.
If you read this and something recently happened in your life, and you feel like you don't want to live anymore, and nothing can change that. Then remember even though it's hard right now, things change in life, and a lot of us have been there. I'm middle age (around 50yo) I have been there two times in my life. It was so painful, and I was sure my life was destroyed. I choose to talk to someone, and since then I've had a lot of happy moments in my life. If you have struggled for a very long time and see no other way, have you really tried everything? could a last solution be to sell everything and travel? or do other radical changes in your life, since you have nothing too loose? Please try EVERYTHING before you make this decision.

I see this commented as a risk, is it really a risk or is it rather people who remove the bag in panic before they lose consciousness?

Let us talk about solutions to prevent this. I can't find a good solution on how to tie hands. Hyperventilating - breathe out - pull down bag - short amount of time to tie hands before breathing in. I know about the Megathread for exit bag with inert gas, but I thought we could discuss and find a solution to prevent removing the bag. I see this as the largest risk of failure with this method. So the things we discuss in this thread is both if there is an actual risk of pulling off the bag, and solutions to prevent it.

I am new to this site and working towards ctb. Would some kind of large elasticated band facilitate the immobility of hands ? Just a thought.

This is from 2006 Guide to Self-Chosen and Humane Death but other sources may vary

"Witnesses report muscular contractions (clonus) in about half of helium deaths. These spasms occur between 2 minutes and 8 minutes into the procedure. Arms and legs will tighten and relax a few times, from about 10 seconds to 2 minutes duration. An inexperienced observer may misinterpret arm-tightening contractions as an effort to remove the bag. This is not the case. These contractions are also common during anesthesia in surgery settings. Witnesses who have attended several helium suicides say that they have never seen a case where the hands have actually reached as far as the plastic bag. They also report that when family members or loved ones are told about these muscular contractions, they are not troubled when they see it happen. Because the patient is unconscious at this point, there will be no experience of discomfort."

Yes it is possible you will remove the bag in a semi conscious state. The devil could possess you, and tell you to stay alive to suffer and make others suffer. Suicidal people are hated by society often, because they are not liked.
