When I picture a partial hanging attempt with my knees on the ground and my body leaned forward into the rope, I totally see how my body could thrash, forcing my neck out from the rope.

Is this a actual issue, and if so, how can it be prevented?

Yes it's a risk, and could result in a vegetative state. The benefit with partial is of course the fact that you are able to do it without a lot of pain. Full suspension (no drop/short drop) removes this risk, but can also result in a lot of pain for minutes, which scares me too.
