I know it may be ignored anyway even if I have a legit form with me, but it can't hurt to have a legit DNR to cover my bases. For my state, I need a physician, APRN, or PA to sign, date, list address, phone #, and licence number as part of the form . Is there a resource or can anyone PM if they know someone that would fill that out and mail back to me? Kind of like how A is a medical professional that helps us, I was hoping we had someone to help with this. But they would need to have a licence to practice in the US to make this valid.

Check if your state allows DNR wrist straps, some places (like Michigan) allow that as an alternative to filed paperwork. If it includes a few proper pieces of information it's valid regardless of if you filed anything else. Also for Michigan if you show ANY vital signs they can attempt to save you regardless, so if you're 8 hours into an SN CTB and still have vital signs they will bring you back.
Just something to think about, find the specifics for your state and do the homework first. Or do something sneaky and find the rules for a neighboring state and if they are better do it there.

Thanks dreamcatcher! Yeah for my state (Delaware) there's a standard form called the DMOST that is supposed to be visible. I ordered a bracelet and necklace online and I'm going to sharpie "DNR" on my forehead and arms and probably forge the form and tape it to the door. From what I read, paramedics are going to do what they do regardless anyway. I don't blame them, it's hard to imagine being able to save someone else's life and not doing it, especially if youre passionate enough about saving lives to do it on a daily basis. I just don't want to take chances. I most likely won't be found alive anyway, just really don't want to be "saved" just in case.
That's really rude to mock another member like that in this of all places. If someone bothers you so much you put them on the ignore list then don't dredge their name up later. Treat others as you wish to be treated.

I always struggle with the concept that I should need a medical practitioner's blessing to get a DNR (or do anything else). As a healthy 24 year old male, there's no way they'd sign off. Just another way the lawmakers and healthcare providers are paternalistic bastards

If all else fails, when you go to ctb, you could always write "Do not revive" on your arms and chest. I heard of a story where someone had it tattooed on their chest, and even though the person didn't have an official form, the hospital obeyed his tattoo/informal request. Definitely worth a shot.

That is an urban legend repeated in many forms over many decades. If there isn't an official form and there is any chance you aren't DEAD dead they will attempt. No tattoo will be respected. Even a proper form that has any doubt/inconsistencies will be ignored. Best you can do unless you can convince a doctor to sign an actual DNR is get a normal Living Will/Advanced Directive that prevents life support etc.
Then as I said ignore and move on if you don't want to deal with it. Dragging it back up to take a cheap shot isn't cool.

Have you had a physical lately? The last one I had they made sure to ask if I had a living will, advanced directive, and if I wanted to file a dnr. I didn't fill it out but thought it was cool they asked. Apparently that's part of being mid-30s now.
I doubt they'd have let you file an actual DNR...for that specific part you have to have a terminal condition or other justification (like being super old etc) generally. It's not something a person who has no impending reason (and good luck selling suicide as one) for it can just have on file. The Living Will/Advanced Directive part is available to any adult and you don't have to explain or justify it.

I have multiple chronic illnesses and have been in a medically induced coma as a result from one, I suddenly get asked by every specialist if I want one since, it just surprised me my general practitioner asked. And I'm waiting on my second cancer diagnosis of my life, if that ends up how I think it will, I'm signing.
Ah well that's clearly the justifiable sort of basis I meant. Your post didn't mention that and I didn't want people thinking you can just get one from your doctor as a matter of routine.

If your state is draconian then find another nearby that has easier requirements and do it there. Although it would be ideal not to be found during your attempt, at least if you follow that it's another chance you'll get to CTB without interference.
Delaware looks to be particularly bad in this case, a form that requires a doctor signature and a good reason? I'd skip even trying to get the paperwork done in that state. And they don't accept DNR bracelets. Maryland next door really isn't much better.

I think for me I am SOL on getting a legit signature but I did print the form from the state website and took it to Staples and had it put on yellow paper like they actually use in the hospital. It looks real.
I imagine it will not be an issue of getting found and saved in time anyway as I'm going to check into a hotel room for 2 days with a do not disturb sign. But just in case.
